1. Acceptance and website purpose

These Terms govern use of the Connected Wellness public website. By using the website, you agree to these Terms to the extent enforceable under applicable law. If you do not agree, do not use the website.

The site is owned or operated by Connected Wellness, LLC and provides general information about the practice and services associated with Steven Dellisanti, D.C., an Arizona-licensed Doctor of Chiropractic, License #8438.

2. Website use does not create a professional relationship

Viewing the website, sending an email, leaving a message, requesting information, or discussing possible services does not by itself create a chiropractor-patient, practitioner-client, fiduciary, or other professional relationship. A professional relationship begins only after the practice accepts the person for services and any required intake, informed-consent, privacy, financial, testing, or other professional documents are completed.

If a separate signed agreement, informed consent, privacy notice, financial policy, laboratory consent, or patient document conflicts with these website Terms, the separate document controls for the professional service it governs.

Website descriptions are general and do not promise that a particular service is appropriate, available, or lawful for every person or in every location. Chiropractic care is provided in person in Prescott, Arizona. Virtual nutritional consultations may be available depending on the person’s location, the nature of the requested service, professional scope, and applicable requirements. Access to the website from a particular jurisdiction does not mean that every service may be provided there.

Genomic testing and analysis, when used, are one source of information within a broader individualized process. Genetic and genomic information may be probabilistic and is limited by the test performed, sample quality, laboratory methods, variants analyzed, current scientific knowledge, and interpretation. To the extent a dispute concerning only use of this public website is subject to a judicial forum and venue may lawfully be selected, venue will lie in the state or federal courts serving Yavapai County, Arizona.

17. Severability, no waiver, and changes

If a provision of these Terms is found unenforceable, the remaining provisions remain in effect to the fullest extent permitted by law. A failure to enforce a provision is not a waiver. Connected Wellness may revise these Terms, and continued use after an update constitutes acceptance of the revised website Terms to the extent permitted by law.
